Hormone Therapy After 60: What Science Actually Says Now

For decades, women were told a hard stop story about hormone therapy. Turn 60. Cross ten years past menopause. End of discussion. That rule is still repeated in exam rooms every day.

And it is not supported by current evidence. For healthy women with ongoing symptoms, continued use beyond age 60 is appropriate when benefits outweigh risks and decisions are revisited regularly. Hot flashes, night sweats, sleep disruption, joint pain, sexual dysfunction, and genitourinary syndrome of menopause do not magically resolve at age 60.

Estrogen protects bones for as long as it is used. When estrogen is withdrawn, bone loss resumes.

A 2024 Medicare analysis of more than 10 million women aged 65 and older found that continued estrogen therapy was associated with: 1.Lower all-cause mortality. 2.Lower breast, lung, and colorectal cancer risk. 3.Lower rates of heart failure, myocardial infarction, and atrial fibrillation. 4.Lower dementia risk.

Hormone therapy decisions for initiating hormone therapy should include a full hormone conversation, not a single-hormone focus.  Estrogen supports bones, brain, cardiovascular health, metabolism, and tissues.  Progesterone is required for uterine protection and plays a key role in sleep and is calming.  Testosterone declines with age and is essential for sexual desire, motivation, energy, muscle preservation, rebuilding bone and overall vitality.

Hormone therapy, when appropriately dosed and monitored, can be transformative for select women well beyond menopause. The research is confirming that aging with hormones is healthier than without.
